Although they appeared to let bygones be bygones by the end of the conversation, Taylor’s mouth seems to have gotten him in trouble with Vanderpump once again. These are supposed to keep order and safety among the migrant workers, but Homer is unaware that they resent and subvert these rules. Along with Homer, the reader comes to realize that the real rules of the Cider House, and of life, are never written down. Homer, the oldest of the orphans, is very bright, helpful and even-tempered, so Larch trains him in obstetrics and abortions as an apprentice despite Homer's never having attended high school. Homer disapproves of abortions, and although Larch has trained him, Homer refuses to perform them. After several years, Homer is very skillful and confident in performing obstetrical duties.

But Homer does not agree with abortion, and he becomes involved with a well-to-do local family of apple growers and the migrant black workers who travel north to work their orchards each year. When Homer reads them the list, the workers observe the rules have been made without the occupants' consent by people who do not live there and do not share their problems. Michael Caine's performance is one of his best, and Charlize Theron is sweet and direct as the girl. Wilbur's and Homer's lives are complicated by the abortions Wilbur provides. Wilbur came to this work reluctantly, but is driven by having seen the horrors of back-alley operations. Homer, upon learning Wilbur's secret, considers it morally wrong.
As a young man, Homer befriends a young couple, Candy Kendall and Wally Worthington, who come to St. Cloud's for an abortion. Homer leaves the orphanage, and returns with them to Wally's family's orchard in Heart's Rock, near the Maine coast. Wally and Homer become best friends and Homer develops a secret love for Candy. Wally goes off to serve in the Second World War and his plane is shot down over Burma. He is declared missing by the military, but Homer and Candy both believe he is dead and move on with their lives, which includes beginning a romantic relationship. When Candy becomes pregnant, they go back to St. Cloud's Orphanage, where their son is born and named Angel.

A subplot follows the character Melony, who grew up alongside Homer in the orphanage. After Homer leaves the orphanage, so does she in an effort to find him. She eventually becomes an electrician and takes a female lover, Lorna. Melony is stoic, who refuses to press charges against a man who brutally broke her nose and arm. She is the catalyst that transforms Homer from his comfortable, but not entirely admirable position, at the apple orchard into Dr. Larch's replacement. The story, set in the pre– and post–World War II era, tells of a young man, Homer Wells, growing up under the guidance of Dr. Wilbur Larch, an obstetrician and abortion provider. Taylor ultimately apologized for claiming the show was scripted in his and his estranged wife Brittany Cartwright‘s podcast When Reality Hits, noting that Vanderpump Rules is “the most authentic” reality show on TV, per ET. The Cider House Rules derives affecting drama from wonderful performances, lovely visuals, and an old-fashioned feel. After Arthur and his team return to the orchard the following season, Homer discovers that Rose, Arthur's daughter, is pregnant.

Each night before sleeping, Dr. Larch says to children "Good night, you Princes of Maine! You Kings of New England!" as both an encouragement and a kind of blessing. The name "The Cider House Rules" refers to the list of rules that migrant workers are supposed to follow at the Ocean View Orchards. However, none of them can read, and they are completely unaware of the rules – which have been posted for years.
